Weeelll... wibbly wobbly timey wimey. Sam (the tall one) was caught in a time loop (like Groundhog day) which ended with Dean (the other one) being killed and then he woke up just to go through the same day again. And no matter how he tried to prevent his brother's death, he couldn't. So Dean dies about 100 times in that episode.
