Help with the refund please

On that form...i had to enter the: Booking Reference Number..



i dont know where to find it...
So i went to ticketmaster...order history...and i entered the order number for my ticket! is this right ? or should i enter another number? maybe the one on my bill for the credit card ? which i throw it away a few months ago
